ABSTRACT

Background: Although there is a growing momentum in Japan to expect residents to play an educational role, the guidelines are still unclear. Objective: The purpose of this study is to examine the characteristics and contents of teaching roles for residents in the new curriculum of each specialty program and uncover current issues. Methods: Two researchers independently searched for and determined the wording related to teaching in the program for all 19 specialties. The research team extracted the relevant issues. Results: Seventeen of the 19 programs included a statement on educational roles for their residents, but almost all of the programs did not include educational competencies and any specific strategy to achieve it. Discussion: The definition of educational competencies and the development of methods to acquire the competencies are issues for the future. It is necessary to develop and expand programs for the Residents-as-Teachers program in Japan.

Abstract Objective To assess the knowledge of patients seen at a teaching hospital about the academic and professional training of the resident doctor in orthopedics and traumatology, as wellas his areaofexpertise, and determinethe perception of thepatients ofcomfort and safety in relation to being assisted by the resident doctor at different stages of treatment. Methods A cross-sectional study was conducted with patients admitted to a large ortho pedics hospital of the Brazilian Unified Health System(SUS, in the Portugues e acronym). Datawere collected through the application of a questionnaire containing 19 objective questions that assessed sociodemographic parameters and the perception of the patient of the performance of the resident. The data were analyzed to assess the frequency of responses obtained. Results 152 participantswere evaluated, predominantlymale(62.6%)andaged between 36 and 55 years old (41.3%). Only 43.3% were aware of the academic background of the resident. Patients reportedfeelingsaferandmorecomfortablebeingassistedbythedoctor together with the resident in the outpatient consultation (43.3%), in the nursing ward (39.3%)andduringsurgery(61%).Asfor theperformanceof theresident,80.2%statedthat the resident doctor improves communication between the patient and the main surgeon; however, only 11% said they would feel safe and comfortable being cared for exclusively by residents in the surgical environment, if allowed. Conclusion The participation of resident physicians in the care is well received by the patients if they are in the company of the attending physician. Patients identify residents as a facilitating bridge in the communication with attending physicians.


Resumo Objetivo Avaliar o conhecimento de pacientes atendidos em um hospital-escola acerca da formação acadêmica e profissional do médico residente em ortopedia e traumatologia, bem como sua área de atuação, e determinar a percepção de conforto e segurança do paciente em relação a ser assistido pelo médico residente em diferentes etapas do tratamento. Métodos Foi realizado um estudo transversal com pacientes internados em um hospital de ortopedia de grande porte do Sistema Único de Saúde (SUS). Os dados foram coletados a partir da aplicação de um questionário contendo 19 questões objetivas que avaliaram parâmetros sociodemográficos e a percepção do paciente quanto à atuação do residente. Os dados foram analisados de forma a avaliar a frequência das respostas obtidas. Resultados Foram avaliados 152 participantes, predominantemente do sexo masculino (62,5%) e com idade entre 36 e 55 anos (41,3%). Apenas 43,3% tinham conhecimento sobre a formação acadêmica do residente. Os pacientes relataram se sentir mais seguros e confortáveis em serem assistidos pelo médico em conjunto com o residente na consulta ambulatorial (43,3%), na enfermaria (39,3%) e durante a cirurgia (61%). Quanto à atuação do residente, 80,2% afirmaram que o médico residente melhora a comunicação entre o paciente e o cirurgião principal, entretanto e apenas 11% disseram se sentir seguros e confortáveis sendo cuidados exclusivamente por residentes no ambiente cirúrgico, caso fosse permitido. Conclusão A participação de médicos residentes nos cuidados é bem recebida pelos pacientes, desde que em companhia do médico assistente. Os pacientes identificam nos residentes uma ponte facilitadora na comunicação com os médicos assistentes.

ABSTRACT

Background: Resident medical officers play a critical role in facilitating conversion of potential deceased donors to actual donors. Since residency is the last opportunity for formal medical training, we contemplated deficiencies in knowledge might originate and a session of sensitization would disseminate updated information about organ donation. This study aims to assess the change in the knowledge and attitude of resident doctors towards organ donation after a programme of sensitization.Methods: A cross sectional study was conducted in March 2018 among newly inducted resident medical officers of KEM Hospital, Mumbai, after obtaining the institutional ethical approval and written informed consent. Using complete enumeration, 75 newly inducted resident doctors were approached out of which 49 consented to participate in the study. A self-administered questionnaire was used to assess the knowledge and perception before and after a programme of sensitization using a pre-test post-test study design.Results: Few numbers (18.36%) of resident doctors were aware of the various terminologies related to organ donation. There are lacunae in knowledge about difference between brainstem death, cardiac death and the organs that can be donated in each case. Moreover, participants who were initially unwilling to pledge for organ donation, were ready to pledge their organs post the sensitisation session (Z=-3.162, p=0.002).Conclusions: Participants knowledge improved over the programme of sensitization.

ABSTRACT

Informed consent remains an important ethical and legal document ensuring autonomy of the research participants. Objective: The present study was undertaken with the idea to find out the gap in respect to knowledge, attitude and practice in acquiring Informed Consent so that we can improve the procedure of acquiring consent in accordance with the present regulations. Methods: Questionnaire based close ended cross sectional study within 1 week time frame in Bankura Sammilani Medical college involving the resident doctors pursuing their post graduate degrees. Results: Though 83.4% had the knowledge but 68% do not feel necessary to take consent. The major limitation being excessive work load and lack of time. Conclusion: The study revealed that there was lot of lacunae in their knowledge giving rise to improper attitude and practice regarding Informed consent procedure in a research project.

ABSTRACT

Objective To explore the application value of ultrasound medical imaging workstation in quality control of ultrasound reports issued by resident doctors.Methods The pancreatic ultrasound reports of outpatients provided by resident doctors with 6 months and 12 months of training were derived from the ultrasound medical imaging workstation as 6-month group (G6M) and 12-month group (G12M),respectively.There were 1582 ultrasound reports in the G6M,including 420 for males and 962 for females,with 1318 for patients < 65 years old and 264 for patients ≥ 65 years old.There were 2723 ultrasound reports in the G12M,including for 760 males and 1963 for females,with 2323 for patients < 65 years old and 400 for patients ≥ 65 years old.Frequency of"unclear tail of the pancreas" in the report,standard image,and body markers were recorded.The chi-square test was used to compare patient gender,age,and the quality of resident reports between the two groups.Results The chi-square test showed that there was no significant difference in sex or age composition between the two groups (all P > 0.05).The number of cases of"unclear tail of the pancreas" reported in the G6M and G12M was 554 (35.02%) and 734 (26.96%),respectively,and there was a statistical difference between the two groups (x2=31.029,P < 0.001).In the reports with "unclear tail of the pancreas",the number of reports with "no map" was 8 (1.44%) and 14 (1.91%) in the G6M and G12M,respectively,and there was no significant difference between the two groups (P > 0.05).As to "map without marker" and "map with marker",there were 337 (60.83%) vs 209 (37.73%) and 308 (41.96%) vs 412 (56.13%) in the G6M and G12M,respectively,and the chi-square test showed that there were significant differences between the two groups (x2=44.960 and 42.834,all P < 0.001).Conclusion Ultrasound medical imaging workstation can be used for periodic spot check,evaluation,and feedback of resident reports,which has a positive role in promoting the quality control of ultrasound reports.

ABSTRACT

Objective To evaluate senior resident training program "resident team leader in the Department of General Internal Medicine" at Peking Union Medical College Hospital.Methods We surveyed the residents or the fellows who had been selected as resident team leaders and received the training from October 2014 to September 2018 on their comments and suggestions.Results Twenty-two rotated senior residents who were selected as team leaders in the Department of General Internal Medicine completed the survey.Almost all(21/22,95.5%) of the respondents reported that they learnt more in general as team leaders by Visual Analog Scale (VAS).The mean VAS scores of clinical skills were 7.23±1.27,7.86± 1.32 in teaching abilities,8.14±0.89 in leadership evaluation.Scales as chief resident assistants were 8.44± 1.26.Sixteen respondents (72.7%)considered that pre-job training by attending doctors was necessary.Another 8 (36.4%)respondents addressed their demands on training of teaching skills.Conclusions The senior resident training program "resident team leader in the Department of General Internal Medicine" improves the competency of rotated senior residents.It is a valuable pilot study on senior resident training and worthy of further application in other departments and hospitals.

ABSTRACT

Objective To explore the issues related to the standardized training of resident doctors at the primary hospital and provide basis for the standardization of doctors' training. Methods Questionnaires were used to investigate the resident titles, ages, and education background in the Huanxian People's Hospital. The self-assessment of the competency of resident doctors and professional contents necessary for their improvement were analyzed to identify the medical topics should be recommended to improve their clinical knowledge and techniques. Results Forty-six of 52 registered residents in the Huanxian People's Hospital participated in the survey with a participation rate of 88.5%. In this hospital, mainly the title of physicians was resident, accounting for 43.3% (52/120), the ages were chiefly in the range of 31 - 35 years, and their education levels were college degree 42.3% (22/52) and bachelor degree 48.1% (25/52). 76.1% (35/46) of the residents found they had difficulties in working and not competent enough on the present post, and 71.7% (33/46) of residents considered that they were in urgent need to strengthen their trainings of clinical thinking and clinical skills. Conclusions Strengthening the residents' self academic qualifications, improving the hospital teaching system, enhancing the teaching management, and focusing on clinical thinking and clinical skills training are the main issues in the current standardized training in primary hospitals. By accelerating the standardized training of residents, they may have the chance to transform to be the middle-level physicians and improve the unreasonable structure of doctor title distribution that will play the key role in the development of primary hospitals.

ABSTRACT

The study investigated the relationship between health information source awareness, use and service delivery of resident doctors in Babcock University Teaching Hospital. The study adopted descriptive research design. Out of the 60 copies of questionnaire administered to all the resident doctors, 52 (87%) were returned and used for the study. The data were analyzed using percentages, mean and Pearson Product Method correlation. The findings revealed there is no significant relationship between health information use and service delivery (r = 452, p < .05). The findings also revealed that the major health information sources consulted in Babcock University Teaching Hospital by resident doctors were medical textbooks, internet and personal collections. The major challenges facing resident doctors in Babcock University Teaching Hospital were non-availability of relevant information, inadequate information retrieval skills, lack of accessibility of sources. The study concluded that there is a need to provide more relevant materials and enhance the internet service in order to improve the quality of service delivery of resident doctors. ABSTRACT

Objective To discuss the significance of social psychological medical humanities pro-fessional skills training in the resident standardization training. Methods Choosing 138 people to attend the training in 17 resident standardization training in Hubei University of Medical,and carrying out medical humanities professional skills training by the form of questionnaire, comparing social psychological profes-sional skills training rules before and after the doctor cognitive situation of the doctor-patient relationship, the grasped social psychological professional skills, doctor-patient communication skills assessment, assess-ment of objective structured clinical examination results and the professional knowledge. Results ①The cognitive situation of the doctor-patient relationship in a survey showed that 56.5% (78/138) patients after training of physicians thought that they are short of doctor-patient communication skills, 66.7% (92/138) believe that good communication skills was the most important skills,differences were statistical significance (P<0.05).②The comparison of the grasped social psychological professional skills showed that 86.2% of the doctors thought inquiry need to listen to every patients,79.8% thought it necessary to build a good commu-nication atmosphere,87.7% thought training could improve the communication skills,79.8% thought resident standardization training was the best stage of social psychological professional skills training,The difference before and after training was statistically significant (P<0.05). ③SEGUE questionnaire scores showed that after training physician training of doctor-patient communication was significantly higher than before,having statistically significant differences (P<0.01). ④OSCE score showed that after training, the total score and each passing rate (60 points) were higher than those before training. ⑤Before and after the training, there were significant differences in physician skill examination in the part of written examination, skill operation and interview score(P<0.05).Conclusions The resident standardization training follows the law of medical education and medical personnel training, strengthens the training of doctors' social psychological skills, and promotes the ability and level of medical service,which is worthy to be popularized.

ABSTRACT

Objective:To carry out the large sample research of the importance of medical ethics in the stand-ardized resident doctor training. Methods:Randomly selected 100 graduated resident doctors from Jiangsu province, Zhejiang province,Shanghai and Jiangxi province of standardization training. Using the questionnaire survey form to investigate the doctor-patient relationship and carry out correlation analysis. Results:Two groups of subjects after research statistics, the analysis found the doctor-patient relationship in the model show the current mutual partici-pation pattern of the more advanced undergraduate group was 22%, 16% in the graduate student degree group has significant difference;While 66% of bachelor's degree group and 70% graduated degrees performance as the guide-cooperation mode. Conclusion:It should strengthen medical ethics in the stage of standardized training, this study further strengthen standardized training for the future stage of low qualification resident ethics application guide the importance and the influence on the doctor-patient relationship in the future physician has provided the basis, at the same time, also provides certain theoretical guidance for doctor-patient relationship training methodology.

ABSTRACT

Resident doctor is the beginning period of a doctor career. Resident standardization training is the basement period for the clinical ability training and is the important part for medical students after graduation education. Vascular ultrasonography belongs to ultrasound medicine and has its own discipline characteristics. The diagnosis accuracy of vascular ultrasonography largely depends on the accumulation of basic knowledge of multiple clinical sciences , standardization of operation techniques and especially the accurate application of hemodynamic theory. According to the discipline characteristics of vascular ultrasonography, we train the residents from the following five aspects: basic theory knowledge, operation techniques, clinic thinking ability, sciences research ability, communica-tion and team work ability. On the basis of practice, we explore the establishment of standardized training mode for medical professionals of vascular ultrasnography in China, training practical, innova-tive and independent talents, to provide reference for improving the professional level of vascular ultra-sound in our country.

ABSTRACT

Objective To explore the application and effect of case-based learning(CBL)in vas-cular surgery clinical teaching. Methods Totally 37 resident doctors were randomly divided into 2 groups respectively: CBL teaching group (n=21)and traditional teaching group (n=16). CBL teaching was con-ducted through the following procedures:selecting typical cases-establishing and applying typical case library-autonomous learning-holding regular seminars. Traditional teaching was conducted through the following procedures: basic theory studying-participating in clinical practice-participating in case discus-sion. Evaluation was conducted based on test socre (written test and clinical operational skill test)and res-idents' feedback of teaching effect. Data were statistically described and independent sample t test was performed. Results Theoretical exam score and clinical skill test score were high in CBL group than in traditional group ((thoretical score:(85.53 ±1.75) vs. (79.94 ±2.29);clinical skill test score:(85.10±1.64)vs.(80.31±1.82)). CBL teaching group had advantages in improving learning efficiency, cultivat-ing clinical thinking,promoting mastery and application of knowledge,broadening knowledge, promoting communication and expression ability and improving study enthusiasm ,et al . Conclusion CBL teaching can effectively improve the teaching quality and obtain higher evaluation. Typical case li-brary should be constantly improved and education of vascular surgical basic theory should be strength-ened to promote CBL.

The dermatolo-gist standardization training mode was set up through tutor responsibility rule , internet learning platform and problem-based learning methods. The teaching level for dermatologists was upgraded by using above methods.

Case based teaching was applied in order to enhance the teaching efficacy and clinical safety for resident doctors in anesthesia training centers and to arouse their learning incentives and improve their clinical performance.Case based discussions on basic cardiopulmonary resuscitation and multiple traumas were conducted.Process consciousness was enhanced and theoretical knowledge analysis was combined with practical clinical skill training for residents in department of anesthesiology to improve the quality of training.

Context: It is now known that resident doctors registered for postgraduate studies are prone to work related stress and eventual burnout. Though stress can happen in any profession, reduced performance of resident doctors due to vocational stress could cause an increase in medical errors and thus affect the quality of life of the patients. Resident doctors at a Municipal hospital in India form a unique population as number of stresses they undergo are many and varied. Aims: To study the prevalence of work-related stress and its anticipated solutions among the resident doctors registered for postgraduate studies in clinical subjects at a tertiary Municipal hospital. Settings and Design: A stratified sampling cross-sectional survey was conducted at the Inpatient, Outpatient, and Intensive Care Units at a tertiary Municipal hospital in Mumbai, India. Material and Methods: Data collection was done using a validated 20-point questionnaire to assess the factors causing stress and their anticipated solutions. Statistical Analysis: Simple percentage analysis of stress questionnaire. Results: 71 resident doctors completed the survey. The major stressors in this cohort were inadequate hostel/quarter facilities (92.1%), and the need to perform extra duties (80.0%). Also, non-conducive environment for clinical training and studies (81.7%), inadequate study (78.9%) and break (81.2%), threat from deadly infections (74.6%), and overburdening with work (69.0%) were the other major stress causing factors. The perceived stress busters were good music (40.8%) and family and friends (40.8%). Eighty-seven percent of the respondents perceived regular physical exercise to be an effective mode of stress management and 83.8% expressed their need to have a simple therapeutic gymnasium established within the campus with a qualified trainer. Conclusions: There is a high level of work related stress among the resident doctors registered for postgraduate clinical studies at a tertiary Municipal hospital in Mumbai. One of the perceived stress busters is regular physical exercise that is structured and under supervision.
